Returning to Day Hab Survey

The Disability Law Center has been receiving reports from day habilitation (day hab) participants and their families who are still unable to return to their day hab programs, or return full time, because of staffing shortages. Often, these participants are people with higher support needs or complex medical needs.

Please help DLC gather information to address this problem. You may answer this survey for yourself or for your family member.
